Fall/slip through the cracks

To go unnoticed or escape attention, often due to a system or process being flawed or inadequate.
The senator expressed concern that some vulnerable families might slip through the cracks of the new healthcare bill. The investigative report found that several safety violations had slipped through the cracks, leading to the accident. Despite the government's efforts, many vulnerable people still slip through the cracks and end up homeless. A 2021 study by the U.S. Department of Education found that 1 in 5 students with disabilities falls through the cracks of the educational system, failing to receive necessary support. The legal system is designed to catch wrongdoers, but some resourceful criminals can still fall through the cracks, avoiding the consequences of their actions.
